Fasting before a blood test is one of those instructions that sounds simple but generates a surprising number of questions. How long exactly? Does black coffee count? What if I forgot and had breakfast? This article answers the most common questions we hear at Pulse Path Lab Core from patients preparing for lipid profiles, glucose tests, and other fasting panels.

Which Tests Require Fasting?

Not all blood tests require fasting. The ones that do are those where food intake directly affects the result. The most common are: fasting glucose and insulin (used in diabetes assessment), lipid profile (total cholesterol, LDL, HDL, triglycerides), and some liver function tests where triglyceride levels are relevant. Tests like full blood count, thyroid function, HbA1c, and most immunology panels do not require fasting. If you are unsure whether your specific test requires fasting, call us before your appointment.

How Long Should You Fast?

For most fasting panels, 10 to 12 hours without food is sufficient. This means that if your appointment is at 8:00, your last meal should be no later than 22:00 the previous evening. The fast does not need to be longer than 12 hours. Extended fasting beyond 14 hours can actually affect some results, particularly triglycerides, by causing the body to mobilise fatty acids. A 10-12 hour overnight fast is the standard and is sufficient for all routine fasting panels.

What Can You Drink During the Fast?

Water is fine and encouraged. Staying hydrated makes venepuncture easier and does not affect any routine fasting test. Plain black coffee (no milk, no sugar) has a minimal effect on most fasting panels, but we recommend avoiding it if you are having a glucose or insulin test, as caffeine can affect insulin sensitivity. Tea without milk is similarly low-risk for most panels. Fruit juice, milk, and any sweetened drinks should be avoided for the full fasting period.

What If You Accidentally Ate Before the Test?

If you had a small amount of food or a milky drink within the fasting window, let us know when you arrive. For some tests, particularly triglycerides and glucose, a non-fasted sample will produce a result that cannot be interpreted in the same way as a fasted one. In most cases, the most practical solution is to rebook for another morning. We do not charge a rebooking fee if you contact us before your appointment time. A non-fasted sample is not dangerous; it simply may not answer the clinical question.
